Lost IHSM Membership Card Replacement

If you lost your IHSM membership card, you can order a replacement here or fill out the lost card form at the IHSM Clubhouse. The replacement card fee is $10.00. If you have a card that is no longer working, there is no fee for a replacement. Please email either membershipihsm@gmail.com or fill out the lost card replacement form and check the option for not working or broken. CARDS ARE FOR ACTIVE MEMBERS ONLY. All other requests will be refunded.
